The worst case scenario is that the thread on the case tube has stripped. This is a significant issue as I believe the tube is integral to the case on the SOH, which means you may potentially need a new mid-case. If it's the thread on the crown itself that's cause the problem then it'll be a much easier fix.
It also sounds like you have another issue whereby the crown is turning but the hands are not moving a proportional amount. This could be a stripped gear, or just that the gears aren't engaging properly.
Either way it's a trip to Breitling as none of that is a DIY fix.
May be the same problem for both - a damaged crown stem could prevent threads and gears from engaging properly.
