It's real. Make sure the serial number on the back of the watch (which is not visible in the pictures) matches the warranty book but otherwise I looks good.

Have a look at the 24h hand position compared to the normal hands: The normal hands are at 6.00h (18.00h), while the hour hand is at about 18.20h. A repair seems due...

True.
Navarravw as Alien says even thought the watch is real there is potentially something wrong with it. The GMT hand should be aligned with an hour marker when the hour hand is aligned on the hour. On this watch it is not so it could indicate a problem. Not sure what that would cost to fix but not likely to be extremely cheap.

Never handled a World, but isn't the GMT hand independently adjustable?

It is but only in increments of one hour, as you adjust the GMT hand it jumps around the dial 1 hour at a time.
