Does the accuracy of mechanical watches vary if they are either worn, kept on winders, or kept in box and only wound manually? i.e. a mechanical watch has a lower accuracy when consistently not worn, compared to wearing it daily etc.

I'm not talking about normal variations, but consistent behavior, and also considering normal usage conditions (not worn when skydiving / climbing Mt. Everest or going to the moon)

There will be variations but not because of winder vs. wear etc. It's more to do with amount of reserve, position and the like. Only real consideration is to give a watch a full wind once a month if stored for an extended period.

Personally I've found that most of my watches all gained time at a slightly greater (but also very consistent) rate when kept on a winder for long periods. However as Roff said that's due to the mainspring being kept at a more consistent state of tension by being more regularly and consistently wound, and at a more consistent position/temperature, etc, than you find when using a watch in the "real world" on your wrist. Wearing a watch (and removing it at night, etc) provides considerable variation on all those aspects, and as a result I personally found any gains/loses evened out over time far more than they do on a winder. Just personal experience.

For my case, All my watches have been more accurate when I wear them as compared to if I don't(Based on full wind and accuracy comparison after 1 day).
