Lump charcoal is much better than briquettes because it is 100% charcoal, but it's still just black bits of burned wood. To add wood flavour you need to buy the different woods. You can buy 'shredded' apple, hickory, mesquite, etc wood and then you soak it for about an hour and throw it on top of the hot charcoal. By soaking it first it won't just burn but will smoke for a decent amount of time.

In terms of carcinogens from charcoal, it's definitely something that you need to be aware of, although the issues are really from prolonged / high volume exposure. As long as you use a barbecue in a well ventilated area and let the charcoal get up to temperature (stop smoking) in a chimney before adding food then there isn't much risk.

And of course, keep your grills clean. Any charcoal or greasy black remains are also carcinogen.
